In this episode, I discuss a medication known as spironolactone (Aldactone).

Everybody welcome back to drug talk as always i’m your host gary campbell today we can be discussing a medication known as spironolactone its brand name is el dark tone and before i talk about the medication itself just keep in mind that this channel is for information purposes only and not to be used as a source for recommendations for your personal healthcare

So spironolactone is a reno competitive aldosterone antagonist it inhibits the effects of aldosterone by competing with aldosterone dependent sodium potassium channels in the distal tubule this would lead to an increase in excretion of water and sodium and it would decrease the excretion of potassium in terms of indications for use spironolactone can be used in

Hypertension or high blood pressure as an add-on therapy this medication can be given to individuals who have hyperaldosteronism it can be used in class 3 and class for heart failure around a lactone can be used to treat edema and it can also be used to treat edema associated with cirrhosis of the liver now before somebody was to use spironolactone there are some

Contraindications that they must clear as well as some precautions and warnings that they should be made aware of individuals with addison’s disease would not be able to use 4 on a lactone this medication cannot be given with a player known and it would also be contraindicated to be used in individuals who have hyperkalemia or high potassium levels now in terms of

Precautions spironolactone is on the biers criteria which is the list of medication of the elderly population should either avoid or use cautiously in the elderly population we would want to avoid giving this medication to individuals with the creatinine clearance less than 30 mils per men because they may be an increased risk of experiencing hyperkalemia or high

Potassium asymptomatic hypouricemia may occur and in some cases it could precipitate gout symptomatic dehydration hypotension or low blood pressure as well as worsening renal function may occur in individuals using spironolactone patients who are salt depleted or patients who are using an ace inhibitor or angiotensin ii receptor blocker may be an increased risk

Of experiencing these effects gynecomastia may occur in individuals using spironolactone and this is typically reversible the risk of this would increase as the dose of the medication increases it would be important to note to some patients at hyperglycemia or high blood sugar may occur and finally four precautions worsening of renal function may occur and this

Would be more of a risk to patients who are using other nephrotoxic drugs such as nsaids now once somebody is cleared of the contraindications and made aware of the precautions and warnings and they start to use spironolactone they can expect to receive their dose orally if somebody is using this medication as an add-on therapy to treat hypertension or high blood

Pressure they may use 25 or 100 milligrams daily in one single or in divided doses this dose can be taken with or without food if any dose adjustments need to be made typically they would make these dose adjustments in two week intervals if somebody is using this medication to treat edema associated with cirrhosis of the liver they may use 100 milligrams initially

And the dosing range would be 25 milligrams to 200 milligrams thereafter this dose may be given as a single dose or may be broken up and spread out throughout the day we would want to give the medication for at least five days before we increase the dose and again the medication can be taken with or without food in this setting now as with all medications there

Are some side effects or adverse reactions that patients may experience while using sparano lactone so i’ll go over some of those here now diarrhea nausea and vomiting are possibilities with this medication we also mentioned gynecomastia which would be the development of breasts some patients may experience somnolence while using spironolactone and menstruation

Disorders as well as erectile dysfunction are also possibilities now some more serious side effects but more rare side effects would be some dermatological side effects such as steven johnson syndrome or toxic epidermal necrolysis hyperglycemia or high blood sugar or hyper amia high potassium some patients may experience low magnesium levels or low sodium levels
